Community Health Worker Partnership For Behavioral Health Equity Program

The Community Health Worker Partnership For Behavioral Health Equity Program receives specialized training and up-to-date health information to engage Albuquerque communities in conversations to promote positive health outcomes in disadvantaged neighborhoods. 
The program also focuses on raising awareness for and destigmatizing behavioral/mental health issues. Because Community Health Workers (CHWs) tend to come from the predominantly communities they serve, they are ideal messengers to deliver rapidly changing health information. 
CHWs maintain the health of Hispanic and Latino families by sitting with them and learning about their medical history/health issues. They then offer skilled suggestions and link them with existing community resources to help their wellbeing. CHWs later follow up with clients to check in on their health progress.
